Home Health Scheduler Team Coordinator

Position Overview

Aveanna Healthcare is seeking a Team Coordinator to join our thriving home health care division, supporting clinical operations and management staff. This role is essential to coordinating daily schedules and administrative tasks that facilitate excellent care delivery.

Daily Responsibilities

• Welcome and assist clients, employees, and visitors promptly, ensuring outstanding customer service.
• Provide daily data analysis and validation through the Homecare Homebase EMR system, complying with industry standards.
• Manage clinician schedules, notify physicians about care updates, and handle pre-billing audits.
• Send out patient care instructions and verify all necessary documentation is received, following up with clinicians, clients, and families as needed.
• Handle office communications such as calls, correspondence, and meetings effectively.
• Maintain office supply inventory.
• Offer continuous customer support and assist with staff scheduling, preparation of client documents, administrative tasks, and client file processing aligned with healthcare and government regulations.
• Collaborate closely with various branches and home office teams to ensure smooth coordination and efficient service delivery.
• Participate in staff meetings and work with branch management regularly to achieve performance goals.
• Adhere strictly to HIPAA and data privacy guidelines.

Qualifications

• Proficiency in Microsoft Office Suite.
• Experience in customer service environments.
• Strong written and verbal communication abilities.

Preferred Experience

• Previous experience in home health scheduling.
• Familiarity with long-term care scheduling.
• Demonstrated understanding of clinical assessments, documentation, and compliance with healthcare policies.

Work Schedule

Monday through Friday, 8:00 AM to 5:00 PM, on-site.
