Role Overview
The Care Navigation Coordinator will manage all incoming inquiries to Care Management Operations, delivering timely assistance by resolving basic patient issues or directing callers to the correct department or online resources. Additionally, the role includes undertaking various operational projects related to care management.
Work Schedule
This role operates Tuesday through Thursday from 12:30pm to 9:00pm ET, and Friday from 9:00am to 5:00pm ET.
Key Responsibilities
- Arrange teleconsultations, appointments with Medical Managers, and sessions with Behavioral Care Team case holders.
- Process and transmit documentation for patients subject to regulations.
- Manage resupply of eligible patients’ needs.
- Efficiently execute assigned tasks and projects prioritizing appropriately.
- Support staff with tools in G Suite or Microsoft Office/Office 365.
- Address customer queries and provide problem resolution.
- Collaborate effectively within the team environment.
- Communicate clearly through telephone, video conferencing, email, and text messaging.
Candidate Qualifications
- Bachelor’s degree or equivalent, preferably in a health-related discipline.
- Experience providing customer service in a fast-paced healthcare setting.
- Prior call center experience is advantageous.
- Strong critical thinking and problem-solving skills.
- Exceptional attention to detail.
- Proficient in comprehending system workflows and logical processes.
- Ability to interpret and adhere to standard operating procedures, including HIPAA and security policies.
- Professional communication skills via email and other digital platforms.
- Clear and effective verbal communication with internal staff, vendors, patients, and external parties.
- Capability to maintain composure and communicate effectively in high-pressure scenarios.
- Familiarity with medical terminology and internal process language is desirable.
