About the Role

This position involves managing and assigning patient visits across multiple disciplines such as nursing, physical therapy, occupational therapy, speech-language pathology, medical social services, dietician, and home health aides. The role requires coordinating schedules among providers to ensure optimal patient care, efficient service usage, and adherence to physician instructions.
Key Responsibilities
- Arrange and assign home health visits by various healthcare disciplines to patients.
- Coordinate schedules among multiple providers and disciplines to optimize care delivery.
- Ensure compliance with physician orders during scheduling processes.
Qualifications & Requirements
- High school diploma or equivalent education is mandatory.
- Prior experience in home health, referral intake, or healthcare-related roles required.
- Demonstrated ability to communicate effectively across all organizational levels.
- Strong interpersonal skills are essential.
- Must pass Pennsylvania Child Abuse History Clearance and Criminal Background Check.
- Physical capability to perform job duties is required.
- Ability to work cooperatively as part of a team.
