Healthy Living Coach

Position Overview

The Healthy Living Coach at St. Petersburg Free Clinic supports vulnerable and underserved populations affected by diabetes, helping improve disease management and overall health outcomes through personalized coaching. The role includes in-person and telephone interactions focusing on patient empowerment, motivation, and resource guidance to reduce diabetes risk factors and enhance self-care.

Key Responsibilities

• Work closely with clinical team members and related programs to promote coordinated, team-based patient care, ensuring patients are referred as per grant protocols.
• Educate patients about diabetes, behavior modification, goal setting, and habit monitoring.
• Provide coaching sessions for prediabetic and diabetic patients to enhance decision-making and motivation while tracking individualized goals and progress.
• Instruct patients on medication usage, effects, and self-monitoring tools like glucometers and glucose monitors.
• Identify obstacles that hinder adherence to care plans, aiming to reduce risk factors and disease progression.
• Assist dieticians and healthcare providers by delivering additional health education.
• Manage and document gym pass distribution to qualifying patients.
• Comply with agency and grant regulations, maintaining data and updating databases regularly.
• Enroll patients in grant-funded and community partner programs and oversee related communications and education.
• Support daily clinic operations, including patient check-in/out and room management alongside the Clinical Manager-Nursing.
• Collaborate with Prescription Health Program and clinical staff to address patient needs, and maintain comprehensive electronic health records (EHR).
• Monitor grant finances and supplies related to diabetes care.
• Participate in program assessments and improvements, providing detailed reports on patient outcomes with the Director of Contracts & Quality Assurance.
• Help facilitate group classes on chronic disease and diabetes management.

Quality and Safety

• Support back-office clinical tasks as needed.
• Ensure proper disposal of biohazardous material and compliance with all regulatory and clinic policies.
• Provide ancillary services consistent with regulations and healthcare provider requirements.

Additional Duties

• Perform other functions assigned by the Lead Nurse Practitioner and Chief Health Initiative Officer.

Required Skills and Qualifications

• Excellent verbal and written communication capabilities.

• Proficiency in Microsoft Office, especially Excel.
• Comfort with learning new technologies and working with various digital tools.
• Experience with Electronic Health Records, preferably eClinicalWorks.
• Alignment with the mission and core values of St. Petersburg Free Clinic.

Education and Experience

• Minimum education: Associate degree or higher.
• Licensed Practice Nurse (LPN) license is highly preferred.
• Certification as a Diabetic Educator is favored.
• 3 to 5 years of direct patient care experience with adult diabetic individuals.

Physical Requirements

• Ability to remain seated for extended periods working on a computer.
• Capability to occasionally lift items up to 15 pounds.
