About the Role

An exciting part-time opportunity is available for a dedicated and skilled Healthcare Support Worker to join the busy Community Mental Health Team for Older Adults. This role is based in Barrow-In-Furness, England, serving the Furness Peninsula. The team works with individuals experiencing complex mental health needs and dementia, as well as their carers. You will be part of an established multi-disciplinary team, providing care in patients' homes and care homes. Responsibilities

  • Contribute to the delivery of care pathways for individuals with dementia and older adults facing mental health challenges.
  • Execute a variety of therapeutic activities and interventions, as assigned by a Community Mental Health Practitioner, to address the direct needs of service users and their carers in community or other settings.
  • Provide one-on-one support and dedicated time to an allocated group of service users and their carers to foster their recovery and support their continued living in the community.
  • Conduct evidence-based assessments under the supervision of a Community Mental Health Practitioner to aid in care assessment and delivery.
  • Support the Community Mental Health Practitioner in implementing agreed care plans, monitoring patient progress, and delivering specific interventions.
  • Assist the Community Mental Health Practitioner in monitoring the physical health of service users.
  • Maintain accurate patient records in compliance with Trust policies and procedures.

Requirements

  • Ability to work effectively both independently and as part of a team.
  • Commitment to working with older adults with mental health needs.
  • Ability to travel independently to various locations, including patients' homes and care homes.
  • Proficiency in recording patient information accurately according to Trust policies.

Additional Information

This is a part-time position and is not eligible for sponsorship. The role requires independent travel within the community.
