About the Role
We are offering an opportunity to join The Meadows, a 24/7 inpatient service with 24 beds, dedicated to caring for individuals over 65 experiencing dementia or organic mental health conditions. Located on the former West Park Hospital Site in Epsom, this busy unit focuses on holistic assessment and treatment for older adults in the acute phase of illness.
Key Responsibilities
- Provide high-quality physical and psychological care according to clinical care plans developed by the Multi-Disciplinary Team (MDT).
- Support individuals in daily living activities such as hygiene, nutrition, clothing, environment upkeep, elimination, rest, and activity.
- Observe, report, and document changes in patients' physical, mental, and behavioral health.
- Maintain professional and therapeutic engagement with service users, offering social activities and emotional stability while respecting professional boundaries.
- Assist in the assessment, planning, implementation, and review of patient care.
- Ensure clear communication by telephone and in-person with patients, carers, and colleagues.
- Adhere to the principles of the Mental Health Act in care delivery.
- Provide escort duties under nurse supervision and participate or co-facilitate therapeutic and social group activities where required.
- Perform prescribed physical observations, including blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight, and height, reporting findings to the nurse in charge.
- Collaborate professionally with staff, other healthcare professionals, and external agencies to promote recovery.
- Participate actively in team meetings and ongoing professional development.
