About the Role

The Health Service Executive (HSE) West and North West invites applications for the permanent Grade VII Case Manager position in Disability Services, based primarily at Merlin Park Campus, County Galway. This role provides a key opportunity to contribute to disability services across the regions of Roscommon, Mayo, Galway, and adjoining counties, involving significant multi-disciplinary team collaboration.

Key Responsibilities

The Case Manager will be responsible for case management activities including placement coordination, discharge liaison, managing packages of care, and maintaining active communication with voluntary organizations. Candidates should have substantial experience in disability services or related fields and demonstrate expertise in case management processes important to this role.

Job Details and Location

The position is permanent and full-time within the HSE West and North West catchment, which includes Donegal, Sligo, Leitrim, Roscommon, Mayo, and Galway. While the immediate vacancy is based at Merlin Park Campus in Galway, the panel established through this competition may serve to fill future full or part-time permanent or specified purpose roles in the region.
