About the Role

This opportunity is for an experienced professional to become the Founding Head of Care Delivery at a well-funded health-tech startup based in New York. You will lead and build the foundational operations for delivering care, focusing on seniors and their families by managing complex care needs, developing scalable processes, and establishing an efficient care management system.

Key Responsibilities

• Manage the initial group of patients directly, ensuring coordinated care and addressing care gaps for elderly clients and their families.
• Develop the operational system for care navigation, including setting protocols, escalation procedures, quality assurance measures, and crafting the care delivery playbook.
• Recruit, train, and oversee the care navigation team while tracking essential performance indicators such as patient enrollment and satisfaction scores.

Ideal Candidate Profile

• Demonstrated expertise navigating complex health or eldercare systems, ideally with backgrounds in clinical nursing or social work (RN, LCSW, SW credentials preferred).
• A strong entrepreneurial spirit with proven success building programs or processes from the ground up amid uncertain or high-pressure environments.
• Exceptional empathy and patient advocacy skills paired with operational rigor to design scalable, effective care frameworks.
