Role Overview

This leadership position is responsible for creating and expanding a high-functioning Community Health Worker (CHW) division within a healthcare organization based in the United States. The role emphasizes operational excellence, workforce development, and enhancing access to culturally appropriate care for underserved populations.

You will direct geographically distributed teams, implement scalable processes, and foster strong collaboration across multidisciplinary care providers. Key Responsibilities

• Oversee CHW operations across various markets including workforce planning, resource deployment, productivity, and performance enhancement.
• Set operational protocols for home visits, community engagements, member outreach, and accurate field documentation.
• Create management systems that emphasize coaching, accountability, quality enhancement, and professional growth within CHW teams.
• Collaborate closely with clinical and care teams to integrate CHWs effectively into comprehensive care models.
• Track operational performance metrics and identify strategies to boost member involvement, care coordination, retention, and health outcomes.
• Support market growth by developing scalable operational frameworks while ensuring service quality and uniformity.
• Mentor frontline managers fostering a culture of responsibility, cooperation, and ongoing improvement.
• Champion process innovations to improve the efficiency and scalability of community healthcare programs.

Candidate Requirements

• Minimum of 8 years experience leading healthcare operations.
• Proven leadership in managing CHWs, care coordinators, patient navigators or related community healthcare workforce.
• Experience directing decentralized field teams across multiple locations.
• Deep understanding of Medicaid beneficiaries, social determinants impacting health, and barriers to access.
• Track record of cultivating frontline leaders and enhancing operational efficiency through well-defined processes and accountability measures.
• Excellent interpersonal skills including communication, coaching, stakeholder engagement, and change management.
• Preferred experience with Medicaid programs such as ECM, Community Supports, CalAIM, or similar care management models.
• Valued experience working with culturally diverse and historically underserved groups.
• Advantageous experience in scaling healthcare operations in fast growth phases.
• Strong preference for bilingual Spanish/English candidates.
