Role Summary

The Data Entry Clerk will work within the Enhanced Care Management (ECM) - Community Supports program, which targets comprehensive and coordinated healthcare services for high-need, complex patient populations. This role plays a crucial part in the program’s data management, enabling optimal care delivery, compliance, and quality assurance.

Key Responsibilities

  • Precisely input ECM student and member demographic details, referral data, admission dates, and mandatory program fields into the electronic health record (EHR) system.
  • Upload essential documentation including consents, eligibility verification, releases of information, care plans, assessments, and outreach logs into accurate EHR sections while maintaining strict filing conventions.
  • Track and address incoming referrals promptly in accordance with MCP timelines.
  • Collaborate with accounting to prepare reports from the EHR system to support timely claims submission and reimbursement.
  • Ensure all admissions and enrollment data comply with Medi-Cal, DHCS, and various managed care plan requirements.
  • Communicate with case managers and clinical staff to resolve documentation inconsistencies and to secure missing paperwork.
  • Support quality assurance audits by ensuring all required documentation is uploaded ahead of internal and external audits.
  • Monitor workload dashboards and notify leadership regarding any delays or issues in referral data processing.
  • Respond professionally to phone and email communications and participate in trainings and meetings as directed.
  • Perform other assigned duties as necessary.

Required Qualifications and Skills

  • High school diploma or GED with at least two years’ experience in data entry or administrative functions, preferably in healthcare or social services.
  • Exceptional attention to detail and ability to accurately handle complex information.
  • Strong organizational and time-management capabilities.
  • Proficiency in data entry and usage of EHR platforms such as Exym or Epic, with capability to quickly adapt to new systems.
  • Good command of Microsoft Office applications including Word and Excel, and familiarity with digital document management.
  • Effective written and verbal communication skills.
  • A valid Driver’s License is mandatory.
  • Experience reviewing services prior to claim submissions in behavioral healthcare settings is advantageous.

Physical and Mental Work Requirements

  • Ability to frequently lift or move up to 50 pounds.
  • Regularly required to sit, stand, type, and use repetitive motions over long periods.
  • Occasional requirements include reaching, climbing, pulling, pushing, stooping, and walking.
  • Must communicate effectively both verbally and in writing, utilizing various types of vision including close and peripheral.
  • Adaptability to shifting priorities, maintaining professionalism, and managing mental and emotional stress.

Work Environment and Conditions

  • Indoor office setting with moderate to occasionally loud noise levels.
  • Temperatures range usually moderate to cool, sometimes experiencing hot or cold conditions.
  • Local travel by vehicle may be required.
