About the Role
This position is a hybrid commercial role that combines care-sector knowledge with business development. Rather than a traditional hard-selling approach, the company is seeking someone who can build trusted professional relationships and act as a clinically informed liaison.
You will focus on creating referral channels by developing strong working relationships with hospital discharge managers, medical social workers, nursing home directors, and similar healthcare decision-makers across Ireland. When referrals come in, you will support families through the sales journey and help finalise the service agreement in a smooth and reassuring way.
Key Responsibilities
- Build referral pipelines by travelling to and connecting with hospitals, primary care settings, and step-down units across the country.
- Become a dependable contact for discharge planners and medical social workers who need premium homecare options for patients.
- Handle warm and inbound leads, then convert interested families into private care agreements.
- Use HubSpot consistently to record site visits, referral origins, and family consultations so that opportunities are tracked properly.
- Work closely with internal operations teams to make sure the agreed care plan is delivered after discharge.
- Educate partner facilities on service capabilities, care standards, and turnaround times so the company is viewed as a reliable discharge solution.
Candidate Profile
- A minimum of 3 years of experience in care management, care coordination, medical social work, nursing, or a similar field is preferred.
- You should understand homecare delivery, patient acuity, and the discharge process in a healthcare setting.
- A proactive business-development mindset is essential, with confidence to start conversations, maintain a sales pipeline, and discuss pricing with families.
- Existing relationships with discharge teams, social workers, or public health nurses in Ireland would be a major advantage.
- You must be able to switch communication style between clinical, efficient discussions with professionals and warm, empathetic conversations with families.
- The role requires willingness to travel nationwide and manage your own schedule and CRM administration independently.
- Comfort with HubSpot and other digital tools is important for tracking performance and managing activity.

Working Arrangement
This is a full-time role with a hybrid working setup. The position is mainly remote, but it includes field-based business development. The expected pattern is 3 days on the road while building a strong referral pipeline, reducing to 2 days per week once the pipeline is established.
Pay Structure
The role includes a guaranteed base salary of €35,000 to €40,000, with uncapped commission. Total expected earnings are €55,000+ per year.
