Position Overview

This role involves two available openings for a Coordinator Special Programs Health Services II within the Howard County Health Department located in Columbia, MD. The position functions primarily as a liaison and lead coordinator for Community Health Workers (CHWs) engaged in outreach and wellness programs across targeted zip codes. Through collaboration with community partners, the coordinator supports activities aimed at reducing health disparities by facilitating community connections to appropriate health services.

Key Responsibilities

• Develop and maintain collaborative relationships with community organizations and internal partners to promote CHW and health department programs.
• Communicate effectively across multiple stakeholders including management, staff, community partners, program participants, and the general public.
• Create and deliver engaging presentations to community groups representing the Community Wellness Program and health initiatives.
• Conduct individual outreach in venues such as libraries and community centers to assess social determinants of health (SDOH) and refer individuals to relevant resources.
• Perform follow-ups with community members to confirm successful service connections and identify emerging needs and barriers.
• Assist with organization and execution of community events, outreach activities, and surveys to enhance engagement and visibility.
• Manage program coordination tasks including monitoring outreach activities, compiling reports, and ensuring equitable access to county programs.
• Track and report on CHW referrals, activities, and community health outcomes.
• Coordinate logistics for in-person and virtual meetings, events, and educational sessions aligned with program objectives.
• Participate in interdepartmental meetings, coalitions, and councils relevant to community health initiatives.
• Undertake additional duties as assigned such as participating in training, completing CHW certification within one year, involvement in flu clinics, and serving on relevant committees.
• Stay informed of countywide health services and emergency protocols, and contribute to quality improvement efforts within the program.

Qualifications and Experience

• High school diploma or equivalent required, with one year of professional experience assisting in coordinating health-related programs or referring individuals to public and private health resources.
• Applicants may substitute 30 college credit hours for experience requirements.
• Military service in relevant health-related roles can substitute for education and experience on a year-for-year basis.

Preferred Attributes

• Strong oral and interpersonal communication capabilities.
• Familiarity with Howard County communities and the local health system.
• State Community Health Worker (CHW) certification or eligibility to obtain certification.

Employment Details and Benefits

This is a full-time hybrid position based primarily onsite at the Howard County Health Department. Contractual employees working 30+ hours weekly may qualify for subsidized health benefits, with the state covering 75% of medical and prescription premiums. Leave is accrued at one hour for every 30 hours worked, up to 40 hours annually. Additional voluntary benefits are available but paid entirely by the employee.
