This opportunity is shared on behalf of a partner employer that handles applications and the remaining hiring steps. The hiring company is seeking a Community Health Worker in the United States to support patients with complex medical and social needs.

The role combines community-based patient engagement with remote coordination. You will help people access care, understand treatment plans, stay connected to services, and navigate the healthcare system more effectively. Daily work may include phone outreach, home visits, accompanying patients to medical appointments when needed, and helping with telehealth visits. The position is highly collaborative and depends on strong communication, empathy, and responsiveness.

Key responsibilities

  • Reach out to patients with complex health and social needs and help keep them engaged in care.
  • Identify missing care needs and link patients to suitable medical and community support services.
  • Work with multidisciplinary care teams to implement individualized care plans.
  • Support patients through calls, home visits, hospital visits, and escorted trips to appointments.
  • Assist patients in joining telehealth appointments and communicating effectively with providers.
  • Deliver condition-specific education and reinforce treatment instructions under clinical direction.
  • Review medications to encourage adherence and flag possible concerns.
  • Carry out basic clinical and home safety checks, including vital signs and environmental safety review.
  • Share patient concerns with clinical staff and contribute to coordination meetings.
  • Help train and orient new Community Health Workers when required.

Required profile

  • High school diploma or GED is mandatory.
  • At least 1 year of related experience in healthcare, outreach, or social services is required.
  • Current Basic Life Support (BLS) certification is required.
  • A valid driver’s license is required.
  • Strong verbal and written communication skills are needed to work with people from varied backgrounds.
  • Ability to juggle multiple tasks, solve problems, and contribute in a team setting.
  • Comfort working independently in community-based and home-based environments.
  • Good organization and attention to detail are essential.
  • A compassionate, patient-focused mindset with strong interpersonal skills is important.
  • Basic administrative and documentation ability, along with familiarity with standard communication tools, is expected.
