Position: Home Health Clinical Scheduling Coordinator

This is an on-site position located near the Galleria area in Houston, TX (610 and Hwy 59).

Key Responsibilities

  • Manage and update clinicians’ schedules based on authorizations and patient requirements, maintaining clinician availability lists.
  • Act as the main point of contact for clinical team members and administrative staff regarding scheduling matters.
  • Maintain a system to track visit frequencies to ensure accurate scheduling.
  • Collaborate with the Director of Nursing (DON) to ensure clinicians meet productivity standards.
  • Assist with staff schedule adjustments and patient reassignment as needed.
  • Foster and sustain positive working relationships with internal and external stakeholders, embodying the core values of Transition Home Healthcare.

Required Qualifications

  • Minimum of one year experience in healthcare scheduling, specifically within home health.
  • Fluency in Spanish is encouraged to apply.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
  • Experience with Home Care Home Base EMR is preferred but not mandatory.
