Role Overview

We seek a motivated and organized individual who recognizes that exceptional patient experiences begin from the initial point of contact. The Intake Specialist will play a critical role in ensuring seamless admissions processes and collaborative care delivery.

Primary Duties

• Deliver a superior admissions experience to patients.
• Effectively communicate with patients, families, case managers, healthcare providers, and team members.
• Educate patients and families about their home healthcare journey.
• Review patient records and relay accurate information to care teams.
• Act as a consistent point of contact during admissions.
• Coordinate care activities with various team members.

Required Qualifications

• Minimum of one year experience in intake within Home Health or Hospice settings.
• Proven ability to manage high volumes of referrals with strong multitasking skills.
• Experience with insurance verification and utilizing multiple hospital portals is mandatory.
• Proficiency with Microsoft Office applications including Word, Excel, PowerPoint, and Outlook.
• Licensed Vocational Nurse (LVN) qualification is advantageous but not compulsory.
• Familiarity with HCHB Electronic Medical Records system.
