About the Role

The Clinical Scheduling Coordinator position is based onsite near the Galleria area (610 and Hwy 59) in Houston, TX. This role involves managing clinician schedules in alignment with patient needs and authorizations, coordinating between clinical and administrative teams, and supporting productivity goals.

Key Responsibilities

  • Develop, update, and maintain clinicians' schedules based on authorizations and patient requirements while managing clinician availability.
  • Act as the central liaison for scheduling-related communications among clinical team members and administrative staff.
  • Keep an accurate tracking system to ensure correct input of visit frequencies for scheduling purposes.
  • Collaborate with the Director of Nursing (DON) to ensure clinicians reach productivity targets.
  • Coordinate adjustments in staff schedules and reallocate patient assignments as needed.
  • Foster positive working relationships with internal teams and external clients, embodying Transition Home Healthcare's core values.

Essential Qualifications

  • Minimum one year of experience with healthcare scheduling, specifically in home health settings.
  • Spanish fluency encouraged to apply.
  • Proficient with Microsoft Office suite including Word, Excel, PowerPoint, and Outlook.
  • Experience with Home Care Home Base EMR is a plus but not mandatory.
