Department Overview
This role is based within the Colorado Department of Human Services’ Civil and Forensic Mental Health (CFMH), encompassing state inpatient psychiatric hospitals, forensic services, and related mental health residential and treatment facilities. The position specifically operates in the Outpatient Services program, supporting individuals transitioning from state facilities and county jails to private inpatient civil commitment hospitals across Colorado.
Job Location and Work Arrangement
The job is remote with the primary work location at the employee’s residence. However, occasional travel and onsite meetings are required mostly within the Denver metropolitan area. These arrangements may change as needed.
Key Responsibilities
- Conduct evidence-based assessments tailored to individual client needs, considering education, language, and cultural factors.
- Analyze assessment results alongside collateral information to identify appropriate treatment and social service requirements.
- Collaborate with Care Coordination Programs and stakeholders to create case plans ensuring coordinated care and placement acquisition.
- Facilitate necessary referrals and documentation to provide clients with indicated services as per established case plans.
- Coordinate client access to various systems including medication management, mental health and substance abuse treatment, case management, and court-related services.
- Maintain close communication with Care Coordination Programs, jails, hospitals, courts, and other stakeholders to ensure service continuity.
- Act as a liaison between healthcare, behavioral health, social assistance programs, jails, courts, and communities to support client placement and transitions.
- Stay informed about service options across Colorado and develop strong working relationships with related partners.
- Transport clients using state vehicles when necessary for placement engagement.
- Serve as a subject-matter expert on benefits acquisition, overseeing the process from referrals to maintenance, tracking submissions and outcomes for the program.
- Communicate benefits status and implications between program navigators, coordinators, courts, and receiving agencies to ensure uninterrupted coverage.
- Understand and comply with federal, state, and local policies, regulations, and processes relevant to benefit acquisition and service delivery.
- Conduct client and stakeholder meetings across varied locations to support coordinated care.
- Manage a large volume of sensitive and legal documents, completing necessary paperwork to obtain mental health, medical, treatment, and legal records while adhering to privacy and confidentiality regulations.
- Prepare detailed documentation after each client interaction and maintain accurate records for clinical, legal, and programmatic use.
- Support ongoing communication and partnership among jails, courts, and mental health services through documentation and coordination.
- Perform additional duties as assigned to meet agency operational needs, including providing coverage for other roles involving case management, administrative support, or direct client contact.
- May assist with complex care coordination beyond the Consent Decree framework, participate in competency docket activities, and support system-wide initiatives affecting the population served.
Minimum Qualifications and Experience
Applicants must have a minimum of six years of relevant work experience related to the duties of this position or possess an equivalent combination of education and experience totaling six years.
Special License Requirements
- Must hold an independent, unrestricted Colorado license as LCSW, LPC, Licensed Psychologist, LMFT, or LAC, or
- Have a preliminary license such as LSW, SW Candidate, LPC Candidate, Marriage and Family Therapy Candidate, Psychologist Candidate, or Addiction Counselor Candidate, or
- Possess a bachelor’s degree in social work, counseling, psychology, or a related field from an accredited institution. Individuals with a unrelated bachelor’s degree are eligible if they have one year of behavioral health work experience at the bachelor’s level, or
- Hold a master’s degree in a mental health field from an accredited college or university.
The position must qualify for Medicaid billing and deliver behavioral health services compliant with HCPF and CMS standards.
Preferred Skills and Experience
- Background in behavioral health case or care management.
- Experience working with serious mental illness populations.
- Strong attention to detail.
- Excellent oral and written communication abilities.
- Familiarity with jail systems and the criminal legal system.
Conditions of Employment
- Required to complete full background and reference checks.
- Residency in the State of Colorado is mandatory at application and must be maintained during employment.
- Mandatory TB testing, annual flu vaccinations, and other required health tests.
- In-state travel to Denver downtown, CMHHIFL, CMHHIP, and contracted hospital locations may be necessary.
- Valid and unrestricted US and Colorado driver’s license is required as travel for official state business is involved, including operating state or non-state fleet vehicles.
- Motor Vehicle Record checks apply due to vehicle operation responsibilities.
- Disclosure of any prior disciplinary termination or resignation in lieu of termination from state employment is required; failure to disclose may disqualify the applicant.
- Compliance with State Personnel Board appeal processes if application is rejected.
Selection Process
