Job Responsibilities
- Manage case coordination, discharge planning, and follow-up care for patients transitioning from ASAM Level Residential Substance Use Disorder Treatment programs (levels 3.7, 3.5, 3.3, 3.1).
- Oversee a caseload of clients, assisting them in overcoming significant barriers by linking them to appropriate community resources.
- Conduct intake assessments, prepare detailed reports, treatment plans, discharge and continuing care plans, and maintain accurate records within client databases and electronic health systems.
- Coordinate group activities for one or more client groups.
- Generate client referrals and develop aftercare plans while exercising independent judgment within established policies and regulations.
- Report and collaborate closely with Program Directors and Supervisors to ensure quality and compliance.
Qualifications & Experience
- Minimum education: Associate's Degree in a Human Services field required; Bachelor's preferred.
- Experience requirements vary depending on degree: five years with Associate's, three years with Bachelor's, two years with Master's degree.
- Certified Addictions Counselor (CAC) qualification requires five years of experience.
