Overview

Griswold Home Care in Houston, TX seeks a dedicated Care Coordinator to join their team. This full-time role, based onsite Monday to Friday from 8:30 am to 5:00 pm, requires a dynamic individual who thrives in a fast-paced environment and enjoys problem-solving with hands-on client and caregiver engagement. This is not a remote position but offers a rewarding experience supporting seniors and individuals with disabilities to live safely at home.

Key Responsibilities

  • Manage a specific caseload of clients and caregivers, maintaining strong communication and relationship-building.
  • Conduct welcome, quality assurance, and satisfaction follow-up calls to ensure high service standards.
  • Perform client home visits and reassessments when necessary.
  • Address client and caregiver concerns promptly and professionally.
  • Monitor changes in client conditions, consulting with Clinical Director as needed.
  • Keep care plans accurate and current, coordinating with Scheduling Coordinators to guarantee consistent client coverage.
  • Assist with caregiver assignment considering client needs and availability.
  • Document all interactions meticulously in WellSky software.
  • Maintain compliance with HIPAA and agency policies.
  • Participate in rotating on-call duties and other assignments as required.

Qualifications and Experience

  • A minimum of two years’ experience in home care, healthcare staffing, hospitality, dispatch, or comparable customer service-intensive settings.
  • Exceptional communication and customer service skills with the ability to multitask efficiently.
  • Capable of managing multiple phone calls, emails, and priorities simultaneously while maintaining composure under pressure.
  • Detail-oriented with strong organizational skills and follow-through capability.
  • Adaptable to learning and navigating various computer systems.
  • Possession of a valid driver's license and dependable transportation.
