Job Overview

The Behavioral Health Care Coordinator delivers phone-based, goal-focused support services to help participants identify needs, set objectives, and improve their wellbeing. Collaborating closely with participants, this role uses motivational interviewing, evidence-based behavior change techniques, and strengths-based coaching to overcome obstacles and build skills for sustainable progress.

Core Duties and Responsibilities

  • Provide phone support that facilitates participants in recognizing unmet needs, establishing measurable goals, and advancing meaningfully through motivational and behavior change methods.
  • Manage a dynamic caseload by accepting inbound transfer calls and initiating outbound outreach to maintain continuous engagement and support.
  • Assist participants in navigating employer benefits and community resources, empowering them to effectively leverage these supports toward achieving their goals.
  • Conduct thorough quality of life assessments to evaluate participant functioning and barriers, delivering focused interventions aligned with individual goals and coordinating care with relevant resources.
  • Maintain accountability by meeting productivity, quality, engagement, and outcome benchmarks to ensure program success.
  • Ensure timely, accurate documentation adhering to organizational, legal, and privacy guidelines.
  • Participate in shared on-call duties including evenings and weekends as required.
  • Perform additional duties as assigned to support the program.

Qualifications and Experience

  • Master's degree in Social Work, Counseling, Psychology, Health and Wellness Coaching, or a related behavioral health discipline.
  • At least 2 years of experience delivering behavioral health or solution-focused support.
  • Proven competency in motivational interviewing, behavior change methodologies, and brief intervention techniques.
  • Strong abilities in assessment, care coordination, and resource navigation, especially relating to employer benefits and wellness programs.
  • Capability to manage an active caseload, efficiently meet performance metrics, and operate independently in a fast-paced environment.
  • Proficiency using electronic documentation tools and common technology platforms.
  • Experience providing telephonic support services is preferred.
  • Availability to work during specified timeframes: 8:30 AM to 5:00 PM PST, 9:30 AM to 6:00 PM MST, 10:30 AM to 7:00 PM CST, 11:30 AM to 8:00 PM EST.
