Role Overview

An exciting opportunity is available for an Advanced Nurse Practitioner to join the North East Rehabilitation Service. Working within a multidisciplinary team alongside a Frailty Practitioner, the role involves collaborating across professional and geographical boundaries to deliver holistic clinical care.

Working Conditions

• Full-time role, 36 hours per week.
• Position located within the North East Rehabilitation Service.
• Service runs seven days a week, requiring flexible geographic work coverage.
• A full driving license and access to a vehicle are mandatory due to travel requirements.

Key Responsibilities

• Provide comprehensive clinical care, including assessment, diagnosis, and management for diverse patients within the Rehabilitation Service.
• Manage an independent caseload with authority to refer, admit, and discharge patients appropriately.
• Facilitate coordinated care through effective liaison with acute services, primary care, and social work teams.
• Promote evidence-based practice while offering clinical leadership and professional advice.
• Contribute to shaping, implementing, and evaluating care standards across the service.

Required Qualifications and Experience

• Registered Nurse or Allied Health Professional qualification.
• Significant clinical experience post-registration, including senior practitioner exposure.
• Master’s degree in Advanced Practice or progression towards this qualification.
• Demonstrated ability to operate independently and proactively.
• Experience in multidisciplinary and integrated healthcare systems.
• Strong leadership, clinical decision-making, and care coordination capabilities.
