Role overview

Koda Health is seeking a Patient Advocate to support adults living with serious or long-term illness as they work through important decisions about future medical care. In this role, you will use education, empathetic communication, and consistent follow-up to help patients record their preferences and make sure care aligns with what matters most to them.

This position goes beyond standard advance care planning. You will develop ongoing patient relationships, work closely with a multidisciplinary team, and help improve how care is delivered across the program. The company is especially interested in clinicians who are adaptable, curious, compassionate, and motivated to help shape a growing organization.

Key responsibilities

  • Facilitate values-focused conversations that help patients clearly express their priorities, treatment preferences, and goals for care.
  • Maintain strong long-term relationships through repeated patient support and follow-up.
  • Teach patients and families about advance care planning, serious illness, palliative care, hospice, and related support resources.
  • Notice shifts in health status and determine when additional support may be needed.
  • Record patient interactions with precision, clinical clarity, and careful communication.
  • Run an assigned patient panel independently while managing outreach, follow-up, and scheduling.
  • Work with Patient Support Associates and the wider clinical team to create a smooth patient experience.
  • Track program metrics and help improve the efficiency and quality of patient engagement.
  • Share ideas that strengthen workflows, patient care, and team performance.

What success looks like

  • Within 30 days: learn Koda's care model, technology, documentation standards, and patient engagement process.
  • Within 60 days: begin independently managing a patient panel and conducting longitudinal follow-up visits with confidence.
  • Within 90 days: consistently provide high-quality care, meet performance expectations, and contribute ideas that improve the clinical program.

Requirements

  • Background in one or more clinical areas such as social work, nursing, care management, hospice or palliative care, geriatrics, patient advocacy, or a similar role.
  • Experience handling emotionally sensitive conversations, ideally involving medical decision-making, care goals, or chronic illness management.
  • Working knowledge of progressive conditions such as COPD, CHF, CKD, or dementia and how they affect function and decision-making.
  • Experience supporting patients and families over multiple interactions.
  • Strong verbal and written communication skills, along with sound critical thinking.
  • Excellent organization and documentation habits.
  • Comfort working autonomously in a remote setting.
  • Availability to work hours aligned with the Eastern Time Zone or Central Time Zone.
  • U.S. work authorization is required; sponsorship is not provided now or in the future.

Preferred background

  • Bilingual ability.
  • Motivational interviewing experience.
  • Exposure to digital health or startup environments.
  • Experience providing longitudinal patient support.

  • Annual base pay of $75,000 to $85,000, depending on experience.
