- Collaborate with both internal departments and external stakeholders to guarantee high-quality care outcomes.
- Coordinate with intake personnel to effectively match participants with support services.
- Organise schedules, assign duties, and provide training for support staff to fulfil each participant's specific needs while monitoring practices and escalating concerns to management promptly.
- Enhance staff capabilities through coaching, focusing on positive risk management, implementing the NDIS Workforce Capability Framework, and watching for burnout and shift-change risks.
- Partner with participants to select appropriate staff members and review living environments and staff performance collaboratively.
- Promote positive work practices and empower participants by coaching and fostering their capacity, emphasizing participant-led life direction rather than service direction.
- Support participants in building knowledge, gaining experience, and connecting with community resources and wider support systems.
- Guide participants in overcoming challenges and reinforce organisational values and culture in daily operations.
- Assist participants in achieving their NDIS goals while complying strictly with the NDIS code of conduct and principles.
- Champion a respectful work environment committed to zero tolerance for behaviours undermining participant rights and inclusivity.
- Engage continuously with people who have lived experience, valuing their expertise and advice.
- Recognise vulnerable participants or scenarios and support workers in managing risks appropriately, including implementing individual safeguards that balance choice and safety.
- Pursue ongoing self-development through reflective practice, maintaining professional relationships, and updating knowledge of best practices.
- Evaluate participant needs and contribute to the creation, planning, and execution of personalized support, skills training, and development programs.
- Conduct interviews with participants to assess their support needs and challenges comprehensively.
- Monitor and document participant progress and support requirements.
- Refer participants to allied health professionals, specialist services, or other agencies to meet additional health or support needs.
- Assess community resources and requirements concerning participant health, welfare, housing, employment, and training services.
- Compile detailed reports and assist in funding applications to governmental and agency bodies concerning participant support and outcomes.
