About the Role

The Quality Outcomes Specialist (QOS) plays a pivotal role by leading quality and outcome initiatives within their designated region and managing the organization's comprehensive outcomes measurement and reporting program. Key Responsibilities
- Lead educational efforts regarding home health and hospice best practices aligned with regulations and company policies, serving as a key resource for both regional and local leadership.
- Review, analyze, and train branch personnel to ensure clinical documentation is thorough, accurately reflects care provided, complies with regulatory standards, and focuses on enhancing patient outcomes.
- Provide practical recommendations to optimize electronic medical records, including workflow and coordination notes, to enhance documentation quality.
- Utilize strong interpersonal and communication abilities to facilitate desired results.
- Support training and integration of supplementary software applications aimed at streamlining operational workflows and improving care delivery.
- Maintain up-to-date knowledge of evidence-based clinical guidelines, organizational protocols, and policies; collect and analyze data to collaborate with local leaders in pinpointing improvement opportunities concerning metrics such as CAHPS, OASIS QRP, HOPE and HCI, Value-Based Purchasing, Outcome and STAR ratings, documentation deficiencies, and process measures.
- Implement a tiered quality support system for assigned agencies, ensuring regular engagement based on performance tiers: monthly for Tier 1 (highest rating), biweekly for Tier 2, and weekly for Tier 3 agencies.
- Ensure continuity, quality, and safety of services in strict compliance with regulatory standards.
- Provide Quality Assurance and Performance Improvement (QAPI) support to agencies.
- Promote collaboration and communication among clinical and non-clinical teams to achieve organizational goals.
- Cultivate a positive, cooperative work atmosphere incorporating operations, clinical, and quality personnel.
- Advance professional and technical expertise through ongoing education and participation in workshops.
Qualifications
- Licensed Registered Nurse or Physical Therapist, with licenses valid in all states within the assigned territory.
- Valid driver's license for state of residence, dependable transportation, and current auto liability insurance.
- Preferably at least 3 years of experience in Quality Assurance and Performance Improvement (QAPI), clinical leadership, or education within home health or hospice environments.
- Familiarity with OASIS and/or HIS/HOPE data collection is mandatory.
- Experience using Homecare Homebase software is preferred.
- Willingness and ability to travel up to 50% within the assigned region.
- Solid understanding of publicly reported quality metrics and patient satisfaction measures.
- Demonstrated current knowledge of Medicare regulations and Conditions of Participation applicable to Home Health or Hospice care.
- Proficient with Microsoft Teams, Excel, Word, and Outlook, and quick to learn new software tools.
- Superb communication skills and the ability to work well with people.
