Role Overview
We are seeking a committed Intake Coordinator to join our expanding team based at the Broadview head office. This full-time position supports our accommodation and community service sectors, focusing on ensuring timely, person-centred service delivery.
Key Responsibilities
- Handle daily enquiries and service requests efficiently, maintaining a person-centred approach within set turnaround times.
- Manage vacancies, waitlists, and service pipelines effectively.
- Oversee daily monitoring of the referrals inbox to action requests promptly.
- Collect and organize necessary documentation from participants and stakeholders to verify eligibility for SACARE’s services.
- Maintain accurate participant data while ensuring privacy and confidentiality.
- Record and update information in the Client Relationship Management System for weekly reporting.
- Coordinate the preparation and scheduling of service agreements, quotations, community visits (preferably face-to-face), initial support needs, and risk assessments.
- Work collaboratively with SACARE’s NDIS specialists, finance, client services, scheduling, service coordination teams, and accommodation managers for smooth transitions.
- Engage in community information sessions to promote services.
- Participate in networking events and exhibitions to enhance referral sources.
- Build and sustain relationships with stakeholders such as support coordinators, plan managers, hospital discharge planners, social workers, and other referrers.
- Partner with the Business Development Manager and Marketing team to develop targeted campaigns, including electronic direct mails, social media, and networking activities to increase referrals.
Candidate Requirements
- Possess a Certificate IV qualification in Business, Community Services, or equivalent experience.
- Hold valid NDIS Workers Check and Working with Children Check.
- Exhibit strong administrative, organizational, and time management skills.
- Have proven experience in the Disability or Community Services sector with knowledge of NDIS and DHS standards.
- Understand current disability-related legislation, policies, and procedures.
- Ability to work independently, prioritise tasks, and meet deadlines.
- Strong interpersonal skills with the ability to collaborate and influence various internal and external stakeholders, including families and carers.
- Demonstrate high initiative, problem-solving skills, professionalism, optimism, and resilience.
- Excellent communication skills, capable of engaging with people from diverse socio-economic and cultural backgrounds.
- Hold a current unrestricted Driver’s Licence.
- Be fully vaccinated against COVID-19.

This role may occasionally require work outside regular hours and flexibility to operate across various SACARE sites. Candidates must be authorised to work in Australia.
