Overview
This role is based at the MDH Wicomico County Health Department in Salisbury, Maryland. The primary responsibility is to coordinate community services for eligible individuals with developmental disabilities living in Wicomico County to ensure they receive appropriate support.
Qualifications
The minimum educational requirement is a high school diploma or equivalent. Candidates should possess at least one year of professional experience related to coordinating health-related programs or connecting individuals with government and private resources. Substitutions are permitted: 30 college credit hours from an accredited institution can replace the required experience, and military service in health care administration or civil affairs in health-related fields can substitute year-for-year for both education and experience.
Preferred Experience
- Experience in case management using a person-centered methodology.
- Ability to participate in and lead team meetings for assigned clients.
- Competence in preparing and monitoring person-centered plans.
