Role Overview

The Clinical Coordinator at Chalmers Home collaborates closely with the Home Manager and Clinical Nurse Manager to ensure exemplary clinical care and maintain a safe, welcoming environment for both residents and staff. This role involves hands-on leadership in overseeing daily resident care, supporting and guiding Registered Nurses, Enrolled Nurses, and Healthcare Assistants in delivering comprehensive clinical services.

Residents in this facility present varied and complex health conditions, including those requiring hospital-level care. The Clinical Coordinator plays a key role in clinical leadership, ensuring continuous oversight of care and mentoring teams to engage proactively in care decisions that uphold high quality standards.

Working 40 hours per week on a set schedule (Tuesday and Wednesday from 07:00 to 15:00, Thursday to Saturday from 09:00 to 17:30), the role requires flexibility to adapt to operational demands. The Clinical Coordinator also participates in on-call duties, providing essential clinical advice beyond day shifts, supported by a closely-knit clinical team aimed at maintaining robust care delivery.

Additional Role Details

• Must be a Registered Nurse with ready experience in aged or dementia care, possessing a minimum of two years handling complex health needs.

• A Post Graduate Certificate in Nursing is advantageous but not mandatory; a commitment to lifelong learning is highly valued.
• Familiarity with InterRAI assessment tools is preferred.
