Position Overview
This role offers targeted case management for individuals with developmental disabilities residing in Cecil, Harford, and Kent Counties, Maryland. The coordinator will facilitate referrals to relevant services, assist in creating annual Person-Centered Plans, monitor service recipients and those awaiting eligibility, and support transition planning. A strong emphasis is placed on organization and detail orientation to promote community integration and improve quality of life.
Minimum Qualifications
- Education: Completion of high school or equivalent certification.
- Experience: At least one year of professional exposure assisting with coordination of health-related programs or managing referrals to governmental and private service sources.
- Education substitution: 30 college credits from an accredited institution may replace experience requirements.
- Military service: Relevant military health-related commissioned officer or Civil Affairs Specialist experience is considered equivalent on a year-for-year basis for education and experience.
Preferred Qualifications
- Ability to manage multiple tasks and meet deadlines effectively.
- Experience working with the developmentally disabled community.
- Bachelor's degree in a human services discipline.
