About the Role
InnovaCare Health is hiring a Case Manager for its Metro Jacksonville location. This is an office-based role focused on supporting patient care coordination, case management activities, and collaboration with providers, patients, families, and insurers.
General Knowledge and Skills
- Strong analytical thinking and the ability to solve problems effectively are essential.
- Background in chronic condition management and care for higher-risk patient groups is an advantage, along with awareness of community support resources.
- A solid understanding of managed care principles is important.
- Excellent verbal and written communication skills are needed for interaction with patients, doctors, and other partners, including the ability to encourage patient participation in their care.
- The role requires the ability to assess patient and family needs promptly and respond appropriately.
- Knowledge of case management practices is expected, including familiarity with Medicare Advantage plans, programs, and services.
- Experience building cooperative professional relationships with patients, providers, and insurance organizations is valuable.
- Comfort with basic computer use, including Microsoft Office, is required.
- Preferred but not mandatory: credentials such as diabetes educator, nutritionist, social worker, or a related specialty certification.
Required Education, Experience, and Certifications
- A current nursing license in the state of practice or a compact license is required; RN is preferred, though an LPN with managed care experience outside a clinical setting may also be considered.
- Candidates should have at least two years of clinical practice and one year of case management experience, or be willing to work toward and maintain a Case Management certification.
- Ongoing continuing education in case management and any applicable professional licensure is expected.
- Current CPR/BLS certification is required.
Work Environment and Travel
This position is based in an office environment and involves sitting, walking, speaking, and listening throughout the workday. A valid driver’s license is required, and travel is expected for about 30% of the role. Occasional travel for clinic-related activities, including meetings or training, may also be needed.
Physical and Mental Requirements
- Ability to lift up to 50 pounds.
- Ability to push or pull heavy items with as much as 100 pounds of force.
- Ability to remain standing or seated for long periods.
- Ability to use fine motor control to operate equipment or machinery.
- Ability to drive and operate a vehicle safely.
- Ability to understand spoken and written instructions.
- Ability to apply logical reasoning to both basic and complex problems.
- Occasional exposure to communicable diseases or bodily fluids may occur.
- Required immunizations and vaccinations are part of the role requirements.
