Your Role
As a Therapy Continuity Coordinator, you will play a vital role in ensuring patients stay engaged with their therapy after initial treatment. This position involves proactive patient outreach and retention efforts geared towards supporting those enrolled in the internal PAP therapy programs, reducing patient churn, encouraging timely reorder cycles, and ensuring a smooth resupply process.
Reporting to the Customer Success Manager, you will contribute directly to the company's recurring revenue stability by applying compassionate communication and organized operational support within a tech-driven environment.
Key Responsibilities
- Manage the transition phase between treatment setup and long-term therapy resupply in partnership with Sleep Technologists.
- Use AI-enhanced care platforms to facilitate seamless patient engagement.
- Collaborate closely with clinical, billing, and operations departments.
- Track and report on patient engagement and reorder metrics to support scalable business operations.
- Conduct timely and proactive resupply communications with patients.
- Adhere to structured workflows aimed at patient retention.
- Maintain precise records for reorder tracking.
- Communicate empathetically and professionally with patients.
- Identify and escalate clinical or billing issues as needed.
- Support adherence monitoring initiatives to optimize patient therapy outcomes.
Qualifications and Skills
- Minimum of two years’ experience in healthcare coordination, durable medical equipment (DME), or patient care navigation.
- Prior experience with PAP/CPAP therapy resupply is highly preferred.
- Excellent communication skills with empathy and professionalism in patient interactions.
- Ability to work independently in a remote, fast-paced environment while coordinating with diverse teams.
- Proficiency using digital care platforms and healthcare technologies.
- Strong data-oriented approach with attention to task tracking, documentation accuracy, and compliance with structured processes.
- Skilled in proactive patient outreach and ensuring meticulous follow-up.
- A strong commitment to enhancing patient experience, therapy adherence, and continuity of care.
- Operational mindset with a focus on process automation and efficiency.
- Availability to work during Central Standard Time business hours.

Compensation
The hourly rate ranges from $25 to $27, depending on candidate experience.
