About Wider Circle and the Role
Wider Circle is seeking a Care Navigator to become part of our Care Delivery Operations team. This entry-level position involves engaging with patients primarily via phone calls, sometimes in person, to conduct health assessments and intake appointments. Establishing rapid trust with patients, gathering information about various social determinants of health such as nutrition, transportation, and housing, and documenting every interaction accurately are central to this role. It offers a clear advancement path toward the Care Advocate position.
Wider Circle collaborates with health plans and providers nationwide to facilitate community health initiatives. Our model encourages neighbors to support each other’s health journeys, enhancing trust, improving patient experiences, and reducing unnecessary hospitalizations, as supported by published research. Our programs operate across tens of thousands of communities.
Principal Responsibilities
- Conduct Health Screenings and Intake Calls: Engage patients using standardized questions during 15-20 minute calls, fostering rapport for accurate information sharing.
- Appointment Scheduling and Follow-Up: Arrange intake sessions within the Healthie platform and maintain consistent patient relationships by scheduling follow-ups yourself. Verify appointments to sustain high participation rates.
- Resource Connection: Use open-ended questions to assess patient needs and coordinate subsequent steps like doctor visits or referrals to Community Health Workers.
- Accurate Record Keeping: Enter patient data into our systems in real-time during calls, ensure note accuracy, complete all documentation before shift-end while adhering to HIPAA and obtaining consent.
- Goal Achievement: Effectively manage daily schedules to meet objectives, including maintaining at least a 60% attendance rate on scheduled intakes and planning appointments to allow sufficient review time.
Applicant Requirements
- Education: Minimum of a high school diploma or equivalent; higher education in Human Services, Public Health, or Behavioral Health is advantageous.
- Language Skills: Strong preference for bilingual candidates fluent in English and Spanish or another additional language.
- Experience: Prior experience in healthcare, social work, community outreach, or care platform use is helpful but not mandatory. Comfort engaging diverse patients on phone calls is essential, with full training provided.
- Technological Proficiency: Ability to navigate web- and app-based platforms and adapt swiftly in a dynamic work environment.
- Self-Management: Capacity to independently handle tasks, maintain organized documentation, and meet targets in a remote setup with minimal supervision.
- Training Commitment: Completion of a comprehensive training curriculum involving call practice, documentation, and role-plays, achieving an 80% or higher score on the final assessment.
- Background Screening: Successful clearance of drug testing and background checks is required prior to employment.
Physical and Work Environment Demands
- Ability to sit, stand, and move intermittently while managing daily responsibilities.
- Good hearing and vision to effectively communicate with patients via phone and in-person.
- Competency in computer use for data entry during conversations without loss of focus.
- Reliable internet and a quiet, private area conducive to remote patient interactions.
