About the Role
Right at Home Brisbane North and South offers in-home and community-based care aimed at enabling clients to remain safely and comfortably in their own residences and neighborhoods. Key Responsibilities and Skills
- Develop and maintain strong interpersonal communication to build positive relationships with clients, their families, and colleagues.
- Deliver community support services effectively, demonstrating experience or aptitude in community health environments working with diverse groups.
- Apply case management principles including service coordination, record keeping, and adherence to care plans.
- Uphold principles of person-centered care, maintaining professional boundaries, confidentiality, and dutiful care ethics.
- Work autonomously and collaboratively within a multidisciplinary team, utilizing strong problem-solving and organizational abilities.
- Comply with local regulatory standards, including background screenings, vaccinations, and licensing requirements.
- Operate transport independently with a valid driver's license and willingness to travel within local areas as needed.
Qualifications
- Certificate or diploma in community services, disability support, aged care, or related disciplines preferred.
- Demonstrated communication skills and experience supporting community health initiatives.
