About the Role

West Bay RI is seeking a dedicated Support Coordinator who is passionate about empowering individuals and their families to live meaningful and fulfilling lives. In this role, you will assist people in navigating services, building connections within the community, and achieving their personal goals through individualized, person-centered planning.

Key Responsibilities

  • Develop and maintain strong relationships with individuals and their families while crafting tailored Individual Support Plans (ISPs/PCPs) that accurately reflect each person's goals, strengths, and preferences.
  • Coordinate a variety of services and community resources that enhance independence, inclusion, and quality of life.
  • Work collaboratively with individuals, families, Conflict-Free Case Management (CFCM), service providers, and interdisciplinary teams to ensure coordinated, responsive, and effective supports.
  • Regularly monitor services, advocate for the rights of individuals, and assist in navigating evolving needs and available opportunities.
  • Inform and educate individuals and their families about accessible services while maintaining a person-centered approach.
  • Ensure accurate documentation and compliance with both agency standards and Rhode Island regulatory requirements.
  • Engage in continuous professional growth and contribute to a supportive, cooperative team environment.

Qualifications and Experience

  • A bachelor's degree in Human Services, Psychology, Special Education, or a related discipline is preferred.
  • 3 to 5 years of experience working in human services, with a preference for experience supporting individuals with intellectual and developmental disabilities.
  • Familiarity with person-centered planning, support coordination, and Rhode Island service systems is advantageous.
  • Excellent communication, organizational, advocacy, and interpersonal relationship-building abilities.
  • Competency in Microsoft Office, Google Workspace, and electronic record-keeping platforms.
  • A sincere commitment to person-centered care, recognizing each individual's unique strengths, contributions, and potential for meaningful life achievements.

Working Hours and Compensation

This full-time position operates Monday through Friday from 8:30 a.m. to 4:30 p.m. The pay range is $28 to $29 per hour.
