  • Create contingency plans for major transition risks, including triggers, response steps, and escalation routes.
  • Track progress using dashboards and status tools for risks, issues, milestones, and KPIs, and call out delays early with corrective recommendations.
  • Maintain strong documentation such as process maps, SOPs, knowledge transfer trackers, sign-offs, readiness checklists, and closure reports.
  • Assist post-launch stabilization through hypercare governance, performance monitoring, and continuous improvement backlog tracking.
