Role Overview

The Intake Coordinator at the Tribeca Campus is integral to the Choose Help team, delivering faith-informed clinical support that encourages shelter residents to pursue quality, comprehensive care and achieve personal goals. This role supports social workers by coordinating care and managing short-term cases, collaborating with Ambassadors to connect guests to resources, and facilitating referrals to maintain a robust continuum of care both internally and externally.

Primary Responsibilities

  • Establish trusting relationships that nurture guests' motivation for positive change and goal attainment.
  • Link guests with internal residential programs and external viable support systems.
  • Manage a caseload and provide general assistance to guests.
  • Hold regular meetings to reassess guests' needs, facilitate resource connections, and track referrals.
  • Help schedule appointments, assist with transportation, and prioritize guest efforts.
  • Lead or organize wellness and life skills groups at the Day Center focused on topics like emotional health and goal setting.
  • Work with Choose Help staff on various operations including front desk tasks, Day Center activities, guest check-ins, and responses to inclement weather.
  • Maintain accurate and timely guest records, including attendance and incident reports, adhering to data integrity and confidentiality protocols.

Requirements

  • Commitment to and alignment with the organization's Statement of Faith.
  • Minimum of two years’ experience in ministry, nonprofit, or human services roles supporting managers or specialists.
  • Bachelor’s degree in Social Work, Psychology, Human Services, or equivalent education and experience.
  • Knowledge of case management basics and administrative procedures.
  • Competence in using Office software and upholding confidentiality standards.

Preferred Qualifications

  • Certification in Mental Health First Aid.
  • Certification in Non-violent Crisis Intervention.
  • Experience with Motivational Interviewing and Trauma-informed Care.
  • First Aid/CPR certification.
  • Training in Naloxone Administration.

Schedule and Compensation

This position requires working Tuesday through Saturday from 10:00 AM to 6:30 PM, with Sundays and Mondays off. The hourly wage is $26.44.
