Position Overview
We are inviting applications for a part-time Physiotherapist role (22.5 hours per week) within the District Therapy Team at Bolton NHS Foundation Trust. This presents an excellent opportunity for Band 5 therapists aiming to enhance their competencies toward Band 6 responsibilities or for Band 6 therapists looking to expand their clinical expertise in a novel area. The role operates across six neighbourhoods within Bolton, fostering a comprehensive, community-focused service model.
Key Responsibilities
- Collaborate closely with acute healthcare providers, community teams, and social care professionals to deliver high-quality community care aimed at preventing hospital admissions and facilitating discharges.
- Provide assessments, personalized care plans, and management for patients requiring intensive home support as alternatives to hospital treatment.
- Engage in multidisciplinary team meetings, integrating efforts with local authority services to ensure cohesive patient care.
- Work within a multidisciplinary team comprising Physiotherapists, Occupational Therapists, Therapy Instructors, and Assistant Practitioners.
- Participate in service delivery over a 7-day span from 8am to 6pm, with staff adhering to a 1 in 6 weekend rotation.
Candidate Requirements
- Registered and experienced Physiotherapist seeking to develop clinical skills in community or intermediate care environments.
- Possess knowledge or prior experience in community or intermediate healthcare services.
- Exhibit strong communication and organisational abilities, alongside flexibility, enthusiasm, adaptability, and commitment to clinically effective patient-centered service development.
- Ability to work effectively within a team to innovate and deliver high-quality care standards.
Additional Details
- The role supports the neighbourhood service model with six bases across Bolton.
- This is a part-time, onsite position with specified working hours and weekend duty patterns.
