Flex Community Guide - Community Health

Position Overview

Reema Health is seeking a dedicated Flex Community Guide to serve individuals across South Carolina. This full-time, exempt position involves a hybrid work model, requiring a professional who is intimately connected to their local community and committed to improving the health and wellbeing of members with complex social and health challenges. The role demands occasional travel within a 25-mile service area and flexibility for some evening or weekend work.

Key Responsibilities

• Conduct outreach to engage and enroll clients from diverse backgrounds with chronic health and social needs.
• Manage an individualized caseload of 60 to 100 members while providing backup support to fellow community guides as necessary.
• Assess and document member needs, focusing on social determinants of health, and develop tailored care plans.
• Build strong, trusting relationships with members through various communication channels including calls, texts, emails, and in-person visits.
• Coordinate care involving behavioral health, primary care, and other providers, and assist with appointment scheduling and attendance.
• Maintain professional boundaries, ensure confidentiality, and seek multidisciplinary support when managing complex cases.
• Travel locally to meet members in homes, community spaces, hospitals, or other care environments as appropriate.
• Identify community resources, facilitate member access including transportation, and foster partnerships with external service providers.
• Manage workload efficiently to meet monthly targets, documenting all interactions accurately via the Reema application.
• Adhere to HIPAA regulations and confidentiality standards consistently.
• Collaborate with peers in a dynamic startup environment, contributing to team goals and adapting to evolving responsibilities.

Required Qualifications and Skills

• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or a related discipline, or equivalent professional experience.
• A minimum of three years of direct experience with similar populations and knowledge of South Carolina community resources.
• Strong commitment to supporting individuals with complex health needs and belief in the potential for recovery.
• Experience with crisis planning, boundary setting, and safety in fieldwork.
• Excellent critical thinking, time management, multitasking, and strategizing abilities.
• Exceptional communication skills to build meaningful relationships and engage members effectively.
• Competency in developing, adapting, and implementing outreach and care strategies.
• Proficient in Google Workspace tools and comfortable using various web-based applications on mobile and desktop platforms.
• Ability to troubleshoot technical issues independently and escalate when necessary.
• Must be 18 years or older, possess a valid driver’s license, maintain vehicle insurance, and have access to reliable transportation.
• Ability to lift up to 25 pounds occasionally.

Compensation and Benefits

• Annual salary range between $50,000 and $54,000.
