Role Overview

This full-time position as a Care Navigator is based remotely within Canada. The individual in this role will serve as the initial point of contact for patients or their family members reaching out to Sailor Health. Responsibilities include facilitating the intake process with empathy, clarity, and professionalism to support patients throughout their care journey.

The role combines patient assistance, care coordination, and emotional support. You will guide each new patient in finding a suitable therapist and help build their confidence and comfort as they begin their care experience. This position perfectly suits those who excel in both empathetic communication and organizational execution, turning vulnerable moments into instances of trust and reassurance.

Candidate Profile

• Compassionate and clear communicator capable of easing difficult conversations.
• Exceptionally organized and detail-oriented, managing multiple patients’ progress efficiently.
• Comfortable using technology such as Google Workspace, electronic health record systems (e.g., Healthie), and customer relationship management platforms (e.g., HubSpot).
• Strong alignment with Sailor Health’s mission to enhance mental health care access for older adults.
• Relevant experience is beneficial but not mandatory; prior roles in healthcare, customer service, or administrative support are advantageous.

Compensation

The salary range for this position is between 40,000 and 50,000 Canadian dollars annually.
