Position Overview

Join the Fylde Coast Home Treatment Team as a senior mental health practitioner dedicated to supporting individuals experiencing acute mental health crises outside hospital settings. This role focuses on delivering recovery-oriented, high-quality care while effectively managing risk in a community environment. The team operates 7 days a week to provide alternatives to inpatient admission.

Team and Service Development

The team is undergoing transformative changes aimed at enhancing service delivery through close collaboration with local partners and integration across the Urgent Care Pathway, striving to uphold Trust values and achieve service excellence.

Key Responsibilities

  • Conduct timely and effective assessments for individuals with mental health concerns to prevent escalation and unnecessary emergency attendances.
  • Function as a vital member of a multi-agency team, administering the first line of telephone assessments and triage for those likely to present at A&E or other safe locations.
  • Perform comprehensive mental health and risk assessments to rapidly evaluate, care for, and refer individuals exhibiting urgent risk factors.
  • Refer patients appropriately to other clinicians or external services/agencies as needed.
  • Execute the gatekeeping role according to the established Operating Procedure.
  • Provide leadership and managerial support in the absence of the Team Manager.
