Case Manager - Social Work

Overview

This full-time, day-shift case management role is based at Prime Healthcare St Francis Hospital in Evanston, Illinois. The schedule runs from 8:00 a.m. to 4:30 p.m., with rotating weekends.

The position focuses on managing patient care quality and resource use from admission through the full continuum of care. The case manager supports patient advocacy, helps control costs, and coordinates with healthcare providers to ensure care is delivered smoothly and efficiently. The role also involves reviewing admissions and concurrent records in a holistic way to assess medical necessity, intensity of service, and severity of illness.

Education, Experience, and Training

Applicants should have a BSW or another bachelor’s degree in a related discipline. A minimum of 1 year of experience in case management, discharge planning, or nursing management is required. Candidates should also have basic to intermediate computer skills.

Preferred Qualifications

Preference will be given to candidates with at least 5 years of acute care experience. Additional preferred credentials include CCM certification, or the ability to obtain it within 1 year, a current BCLS certificate, and familiarity with Milliman Criteria and InterQual Criteria.

Compensation and Benefits

The estimated compensation range for this position is $25.39 to $36.77. Final pay will be determined at the time of hire based on factors such as skills, relevant experience, education, credentials, and licensure.
