Role overview
This position sits within the Blaenau Gwent Memory Assessment Service and is part of the wider Mental Health Team. The role is suited to a senior nurse who can carry out first-stage screening and provide nurse-led follow-up for people experiencing memory-related difficulties or living with a dementia diagnosis. You will collaborate with a broad range of multidisciplinary teams and external agencies to support effective health and social care delivery, recovery, and social inclusion in the local area. The post also contributes to the ongoing growth of the service and may include supervision of junior colleagues.
Main purpose of the role
You will deliver a person-centred, efficient and cost-conscious service for people referred to the Memory Assessment team because of cognitive changes or concerns. The role calls for a supportive, educational approach when working with service users and their families or carers, along with proactive clinical assessment when required. You will help coordinate timely assessment, discharge and transfer pathways for patients, using strong clinical, managerial and communication skills. The role also involves managing patients with complex health and social care needs linked to memory difficulties.
Key accountabilities
- Provide initial screening and ongoing nurse-led review for people referred with memory concerns or a dementia diagnosis.
- Work collaboratively with multidisciplinary services and partner agencies to support integrated care.
- Deliver care using a person-centred, supportive and educational approach for service users and their families or carers.
- Carry out timely clinical assessment and respond proactively where clinically indicated.
- Coordinate assessment, discharge and transfer processes for patients in the older adult memory pathway.
- Manage patients with complex physical, psychological and social needs associated with memory difficulty.
- Contribute to the development of the Memory Assessment Service in Blaenau Gwent.
- Provide supervision and guidance to junior staff where required.
Contract and service information
This is a fixed-term position for 12 months to meet service needs. The organisation supports flexible working and is committed to maintaining a smoke-free environment across all sites.
