Role Overview

The Healthguide acts as a vital bridge between patients and their primary care physicians or medical practice teams. Supported by clinical staff, the Healthguide works to strengthen patient communication, address healthcare challenges, and guide patients through their care pathways.

Key Responsibilities

• Operate mainly in a remote setting, collaborating with patients' medical staff and Guidehealth’s Case Management team to manage targeted patient populations.
• Work autonomously while maintaining close collaboration as an extension of the medical practice.
• Assist patients in achieving improved health outcomes by resolving their inquiries, facilitating communication with medical staff, implementing impactful care plans, closing care gaps, escalating medical issues, and connecting patients with specialty referrals and community-based organizations.
• Accurately record patients' medical histories and concerns in electronic health records (EHR).
• Understand and participate in referral and prior authorization processes.
• Build and sustain professional working relationships with medical practices and Guidehealth teams.
• Meet assigned Key Performance Indicators aimed at better patient health results and reduced medical costs.
• Engage and schedule high-risk patients using various communication tools such as calls, texts, portals, and emails.
• Escalate medical concerns appropriately to Health Guide Registered Nurses, primary care physicians, or Guidehealth leadership.
• Adhere strictly to security protocols to safeguard all Protected Health Information (PHI) and Guidehealth intellectual properties.
• Travel as required (10-15%) to clinical locations in Atlanta, GA for patient engagement and retinal eye exams, with paid training provided.

Qualifications and Skills Needed

• Certification as a Medical Assistant or EMT with at least two years of relevant experience.
• Passion for coordinating patient care, especially for individuals with complex medical and social needs.
• Willingness to develop skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
• Strong interpersonal abilities emphasizing empathy, patience, professionalism, and respect in all interactions.
• Excellent verbal, written, and visual communication skills, and proficiency in documenting patient interviews accurately via software in real-time.
• Good knowledge of medical terminology and the primary care physician practice environment.
• Understanding of social determinants affecting health.
• Ability to adeptly navigate various technologies, including multiple platforms, EHR systems, care management databases, phone systems, and Microsoft Office.
• Familiarity with electronic health record systems such as eClinicalWorks, EPIC, Athena, and Cerner.
• Strong organizational skills to manage competing tasks with attention to detail.
• Capability to work independently within a patient care team under licensed clinical supervision.
• Experience succeeding in a remote work environment.
