The London Borough of Hounslow is looking to welcome a Rehabilitation Assistant into its Community Recovery Service. About the Role
This is a permanent role for both full-time and part-time staff within the Community Recovery Service. The service is a joint health and social care model delivered with an NHS Trust, designed to help people regain independence after illness, injury, or a change in life circumstances while living at home. The team provides rehabilitation and reablement support through an integrated approach involving health and social care professionals.
The post requires flexibility to support vulnerable residents, their carers, and colleagues across a seven-day service operating from 8:00 a.m. to 10:00 p.m.
The Team
You will work as part of a multidisciplinary group that includes therapists, social workers, and nurses, with support from assessors, coordinators, and the registered manager.
What the Job Involves
- Providing personal care support linked to rehabilitation goals, helping clients complete tasks they cannot fully manage themselves, such as washing, dressing, shaving, continence care, toileting, bathing, transfers in and out of bed, and medication administration. Training will be provided.
- Working with clients referred to the Community Recovery Service in their homes or other community settings, including working independently when appropriate and following lone-working procedures and risk assessment requirements.
- Showing an understanding of how disability can affect daily life, particularly in the context of community rehabilitation.
- Supporting client assessment work when delegated by an occupational therapist or physiotherapist and feeding back on occupational performance.
- Contributing ideas to improve methods of working and service delivery.

- Ability to teach practical skills to clients.
- Basic awareness of the effects of disability.
- Understanding of how enabling approaches support clients through therapy.
- The physical capability to perform moderate to strenuous activity throughout the working day while handling multiple tasks at once.
Desirable Background
NVQ Level 2, a Care Certificate, or equivalent experience would be beneficial. Training will be provided.
Essential Requirement
An enhanced DBS check is required for this role.
