Public Health Specialist I - Chronic Disease

Overview

We are seeking a dedicated and energetic public health specialist to join the Vermont Department of Health's Springfield District Office. This role focuses on population health improvement across 15 towns by addressing chronic disease issues with a special emphasis on health equity. The position involves collaboration with diverse community partners to develop and implement systemic and policy-driven solutions for better oral health, tobacco prevention, obesity reduction, and enhanced access to preventive care.

Key Responsibilities

• Design, implement, and assess system-level strategies to improve oral health outcomes, reduce tobacco use and obesity rates, and enhance access to nutritious foods for individuals of all ages.
• Develop solutions to increase accessibility of preventive health services while minimizing barriers to care.
• Collaborate with employers to foster healthier work environments that yield improved health and economic benefits.
• Provide expertise to integrate health equity and access-to-care initiatives into municipal and organizational plans.
• Support community coalitions by analyzing health data, setting objectives, and crafting plans to advance health outcomes.
• Work with a broad range of partners to implement chronic disease models within the district to create significant health improvements.

Candidate Eligibility

Minimum Qualifications

• At least two years experience in public health, human services, emergency response, community engagement, or related fields; OR
• An associate degree in related fields plus at least one year of relevant experience; OR
• A bachelor's degree in relevant disciplines.
