Role overview
The Population Health Coordinator helps members with lower medical complexity navigate healthcare and social support resources. This position focuses on clarifying each member’s care priorities, linking them to the right internal or external services for specific needs, and supporting work related to health-related social needs, gap closure, and BOI review.
This is a fully virtual position. It calls for strong organization, communication, triage, data handling, customer service, and technology skills, along with the ability to work productively with little day-to-day supervision.
What you will do
- Manage clinical and social support requests for members that come through the queue.
- Check member benefit tools to confirm eligibility when needed.
- Provide administrative follow-through by contacting members and internal teams to help close BOI and quality gaps.
- Receive members from engagement teams and work to understand and address social needs or other care gaps.
- Spot new needs, route members to the right care team resources, support clinical appointment scheduling, and help close referral loops.
- Use care facilitation, EHR, and scheduling systems to record interactions, monitor tasks, and coordinate communication across teams.
Experience and education
The position calls for at least 2 years of relevant experience and a high school diploma.
Compensation and benefits
The expected pay range for this role is $27.40 to $35.10 per hour. Compensation is set based on qualifications, skills, experience, and geographic location.
