Position Summary

The Population Health Administrator will leverage comprehensive expertise in population health management, project leadership, data analysis, and process improvement to oversee high-impact, complex initiatives across the health system. The role is pivotal in formulating, executing, and sustaining system-wide strategies aimed at elevating quality and financial outcomes within Population Health and Value Based Care programs. This position requires collaboration with multidisciplinary teams to build necessary infrastructures and workflows aligning with strategic goals. Responsibilities include independently managing a portfolio of system-level improvement projects from conception through sustainability, utilizing judgment and innovation.

Key Responsibilities

  • Lead and manage complex improvement projects focused on population health and value-based care.
  • Develop and implement organizational strategies to enhance quality outcomes and financial performance.
  • Create and oversee infrastructures and processes to meet organizational objectives.
  • Conduct data analysis and interpret complex reports to identify trends and inform actionable plans.
  • Collaborate with cross-functional teams and stakeholders to ensure alignment and project success.
  • Apply change management and performance improvement methodologies to support initiatives.
  • Prepare and deliver presentations to various audiences, including executives and clinicians.
  • Maintain confidentiality and handle sensitive information with discretion according to policies.

Qualifications and Skills

  • Bachelor’s degree required; Master's degree preferred in a related field.
  • Current California RN license preferred.
  • Minimum of five years of relevant experience in population health, quality improvement, or value-based care.
  • Strong proficiency with MS Office tools, emphasizing PowerPoint, Excel, and Visio.
  • Advanced analytical and critical thinking capabilities to handle complex datasets and develop reports.
  • Demonstrated knowledge of performance improvement standards and value-based care models.
  • Experience with population health program evaluation and implementation.
  • Excellent verbal and written communication skills for engaging diverse audiences.
  • Proven ability to lead teams, manage multiple projects, and maintain focus on deadlines.
  • Resourceful, innovative, and proactive problem-solving skills.
  • Experience with EPIC systems and healthcare data analytics is advantageous.
