Role Overview

The Intake Patient Care Coordinator for the Home Health division plays a vital role in ensuring excellent patient care from the very first contact. This position involves handling referrals by managing phone communications, gathering essential information, completing authorizations, and verifying insurance details. While this role does not involve direct patient interaction, its accuracy and confidentiality help create an environment of trust and quality care.

Responsibilities

• Serve as the initial point of contact for referral sources phone inquiries
• Collect and accurately document necessary patient information
• Process authorizations and confirm insurance coverage
• Maintain strict confidentiality and privacy standards
• Support smooth operational flow to facilitate patient care delivery

Qualifications and Skills

• Preferably 1-2 years of medical office experience
• Strong interpersonal skills to relate effectively with staff and professionals
• Compassionate, service-oriented attitude
• Excellent verbal and written communication skills
• Strong organizational and time management capabilities
• Valid driver’s license, current insurance, and reliable transportation
• Must have a valid Arizona Fingerprint Clearance Card
• Current CPR certification and proof of negative TB test required

Work Schedule and Location

Full-time position, Monday through Friday from 8:00 AM to 5:00 PM, working onsite at 7227 E. Baseline Rd #129, Mesa, AZ 85209.
