Role Overview
Diverge Health is hiring a Medical Records Coordinator to serve as the connection point between primary care, specialty care, and internal clinical teams. This person will manage the process of gathering, organizing, and moving clinical records into the company’s electronic medical record system so patient-facing teams can deliver timely care and support patients in accessing government benefits.
What You Will Do
- Collect provider and practice details through patient conversations and obtain medical records directly from hospitals, physician offices, and other healthcare organizations, or locate them within partner EMR systems.
- Assemble complete patient health histories, including records from primary care, specialists, behavioral health, lab results, and visit notes.
- Review, track, and upload clinical documentation into the Diverge Health EMR so that records remain complete, accurate, and easy for teams to access.
- Transfer and transcribe clinical information from partner EMR systems into patient records in the Diverge Health EMR when needed.
- Follow up with external practices to clear documentation gaps and resolve pending record requests.
What You Bring
- Hands-on experience using one or more EMR platforms and the ability to move efficiently across multiple systems.
- Working knowledge of clinical documentation, medical terminology, or healthcare operations.
- Comfort communicating often with external healthcare administrators and physicians by phone, email, and fax.
- Experience using Salesforce or a similar CRM or internal tracking database.
- Excellent attention to detail and a history of keeping records accurate and organized in a fast-paced environment.
- Persistence when faced with administrative barriers to data sharing.
- Working understanding of HIPAA requirements and how to protect sensitive health information appropriately.
Preferred Background
- Experience in value-based care, care management, or population health.
- Exposure to referral coordination or patient intake processes.

Employment Details
This is a full-time, non-exempt position. The hourly pay range is $26 to $29, depending on experience.
