Role Overview
LaSante Health Center is recruiting a Medicaid Navigator to support patients in accessing vital assistance and programs. The person in this role will perform screenings for social needs, determine eligibility, and connect patients with community resources including housing, food aid, transportation, behavioral health services, and Medicaid options such as those under the 1115 Waiver program. This position values strong interpersonal communication, thoroughness, and a compassionate demeanor toward patient care.
Key Duties
- Conduct screenings of patients for Social Determinants of Health (SDOH) needs.
- Evaluate eligibility criteria and link patients to Medicaid and other local community services.
- Provide clear and empathetic guidance through the range of programs and benefits accessible to patients.
- Work collaboratively with clinical team members and outside partners to maintain seamless continuity of care.
- Ensure precise documentation of all activities within the Electronic Health Record (EHR) system while adhering strictly to HIPAA regulations.
Candidate Profile
- Excellent communication abilities paired with strong customer service skills.
- Proficiency and comfort in using computer systems and EHR software.
- Exceptional organizational skills and meticulous attention to detail, capable of juggling various tasks efficiently.
- Prior experience in care coordination, social services, or Medicaid program navigation is advantageous.
