Position Overview
This remote role focuses on meticulous, detail-oriented review work. The position requires ensuring daily documentation from direct support staff, recorded in an electronic health record system, is complete, accurate, timely, and ready for audits across your assigned caseload.
Core Responsibilities
- Daily examination of shift documentation for residential clients.
- Detection and resolution tracking of absent, late, incomplete, or duplicated records.
- Providing individualized coaching and retraining to staff on proper and accurate documentation, including handling unusual circumstances.
- Collaborating with original service providers for timely corrections and appropriate dating of late entries and addenda.
- Confirming documentation aligns with each individual's Individual Support Plan (ISP).
- Maintaining daily completion logs and delivering weekly analytic reports to leadership highlighting recurring errors and documentation gaps.
- Utilizing reporting and AI-enabled tools to extract, format, and summarize documentation information.
- Preparing and sustaining documentation readiness for audits and licensing inspections.

Minimum Qualifications
- Associate degree or above, preferably in health information, human services, healthcare administration, or related disciplines.
- Hands-on experience with electronic health record systems; familiarity with Practical Health Solutions, Therap, or similar ID/DD platforms is highly desirable.
- Strong written communication skills with the ability to assess note accuracy critically.
- Dependable home internet connection and quiet work setting; availability aligning with Eastern Time business hours.
Preferred Experiences
- Background in health information management (e.g., RHIT or equivalent coursework), medical records, Medicaid waiver service roles (DSP, QIDP, program management, support coordination), clinical documentation review, or quality assurance.

Work Schedule and Compensation
The role begins with approximately 30 hours weekly, with prospects to expand to full-time as duties extend to training development and audit support. The pay scale ranges between $23 and $28 per hour, depending on experience and familiarity with electronic health records. The position is fully remote, with periodic virtual meetings involving program leadership.
